Rights Pertaining to Evaluation and Assessment
Overall your family’s rights pertaining to evaluation and assessment includes the following:
- You Have a Right To give written consent before an agency conducts your child’s first evaluation.
- You Have a Right To notify your Service Coordinator that you with to withdraw your consent before your child’s first evaluation is completed.
- You Have a Right To an evaluation at no cost to you.
- You Have a Right To have an evaluation of your child and family that is directed by you and designed by you and other members of the core team.
- You Have a Right To decide what information about your family you wish to share during the evaluation.
- You Have a Right To have the evaluation conducted by qualified individuals.
- You Have a Right To an evaluation based on your child’s medical history and current health, mental and physical development (including vision and hearing), communication skills, social or emotional development and adaptive skills, such as dressing and eating.
- You Have a Right To an evaluation that includes suggestions and ideas for services that would benefit your child.
- You Have a Right To an evaluation being competed within 45 fays of the time of referral, unless your family needs more time.
- You Have a Right To an interim (temporary) Individualized Family Service Plan (IFSP).
- You Have a Right To have evaluation and assessment procedures explained in your primary language or other preferred mode of communication, unless it is not feasible to do so.
