After your child is found eligible for Early Intervention services, your family will be assigned a service coordinator. A service coordinator is your “contact person” in Early Intervention, and is either a nurse or a social worker. This person literally coordinates your services, and in addition to helping you develop the Individual Family Service Plan (IFSP), is available to answer any of your questions or concerns. Your service coordinator will be with you throughout the entire time your family is involved in Early Intervention, and is also available to provide you with referral and resource information in the community that will help you to best help your child. When your child approaches the age of three, your service coordinator will help you determine what is the next step after Early Intervention and will guide you in the transition.
