MSW and LSW required by grant, to provide case management services for special needs children aged birth to 21 years. Elements of this role include providing access to appropriate levels of care within the community, knowledge of Medicaid Waiver, Developmental Disabilities and referrals to other programs as appropriate.
- Bi-lingual ability and computer skills a plus
- schedule is 35 hours/week, Monday- Friday
